Disease severity ratio in GRACE model

Dear all,
I would like to implement GRACE(The Generalized Risk - Adjusted Cost - Effectiveness) framework in my analisys, however to calculate willingess to pay threshold I need to use formula :

K ω H R = C ω H ⁄ ω C R,
where R is the disease severity ration. My question is - how can you obtain this value?
